POSITION SUMMARY: The primary purpose of the Case Manager is to develop, implement, and evaluate individualized patient care plans that maximize reimburesment to the facility under the various payer programs while providing outstanding quality care to the residents.

KEY JOB FUNCTION:

  • Manage the care provided to achieve expected clinical outcomes and customer satisfaction by ensuring that appropriate nursing, therapy and approved ancillary (pharmacy and laboratory) services are provided.

  • Effectively liaison to balance patient's needs and expectations with managed care provider's expectations.

  • Upon admission initiates and coordinates necessary education required by the resident and caregivers to achieve discharge.

  • Ensure discharge plan is understood and acceptable at admission to patient and to managed care provider.

  • Negotiate adequate reimbursement to facility for 'exception' cases to facilitate provision of proper service delivery.

  • Provide timely, accurate and complete documentation in form required by managed care company.

  • Must be licensed RN or LVN/LPN or BSW.

  • Must have at least 2 years clinical experience in a long term care setting.

  • Must be knowledgeable of nursing and medical practices and procedures, as well as law, regulations, and guidelines that pertain to long-term care.

  • Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

  • Prefer 5 years of clinical experience.
